When walking to my stand around 5 am, I always use my red headlamp. Once in my tree stand and secure I turn it off. I sit there for a little over an hour this time of year in pitch black. If I keep the red light on to see around will it spook deer or will they not be cognizant of it?

I think they may notice it. The only way to see if they are alarmed is try it.
